                 Rates:
Initial Assessment& Treatment:  $125.00
       Follow-up session:  $ 85.00/hour
        1-1/2 hour session:$120.00
Payment is due when services are rendered.

  Integrative Therapy & Wellness does not accept Insurance assignment.  If you would like to get reimbursed from your insurance company, you will need to get a PT prescription from your doctor and we will be
happy to give you a superbill to submit.
